About Transition 


Transition Pathways is a personalized program that helps students prepare for life after high school by building independence, confidence, and practical life skills.

Designed for students with individualized transition goals, the program blends academics, career readiness, independent living instruction, and community experiences to help students successfully move into employment, postsecondary education, or adult services. Students remain connected to the larger Lakeshore School of Innovation community while receiving individualized supports tailored to their goals and needs.

 
What Learning Looks Like


Personalized Academic Learning
Students continue working toward their academic goals through individualized instruction aligned with their IEP and Postsecondary Transition Plan. Learning focuses on building functional academic skills, financial literacy, communication, and self-advocacy while allowing students to demonstrate learning in ways that best reflect their strengths.

Independent Living Skills
Students participate in hands-on learning experiences that prepare them for everyday life, including:

  • Personal budgeting
  • Meal planning and cooking
  • Household management
  • Banking and financial literacy
  • Community navigation
  • Transportation skills

These experiences help students build confidence and independence for life beyond high school.

Career Readiness


Students explore careers through job shadowing, volunteer experiences, Youth Apprenticeships, and community-based work experiences.

They also develop workplace skills through resume writing, interview preparation, communication, teamwork, and professional expectations.

 
Community-Based Learning


Learning extends beyond the classroom through meaningful experiences in the community.

Students build independence by practicing real-world skills while working alongside employers, community partners, and adult service agencies. These experiences help students gain confidence while preparing for employment and life after graduation.

 
Personalized Transition Planning


Every student's transition plan is unique.

Working closely with families, educators, and community partners, students develop individualized goals that support their future plans while strengthening skills in:

  • Self-advocacy
  • Independent living
  • Career exploration
  • Postsecondary planning
  • Community participation

Throughout the program, students receive ongoing support as they prepare for the next stage of their journey.

 
Preparing Students for the Future


Transition Pathways helps students leave high school with more than a diploma.

Graduates are prepared to advocate for themselves, navigate daily responsibilities with greater independence, participate in their communities, and pursue employment, postsecondary education, or adult support services with confidence.
